This article provides detailed information about the subjects required to participate in health insurance according to Vietnamese law. It helps readers better understand who needs to participate in health insurance to ensure health benefits and comply with state regulations.
Pursuant to Decree 146/2018/ND-CP, the groups of subjects participating in health insurance include the following 6 groups:
The health insurance premium rate for each subject is different. In 2023, the premium rate for groups of subjects participating in health insurance will be implemented according to the provisions of Article 7, Decree 146/2018/ND-CP and Law on Health Insurance 2014
For employees and businesses, the health insurance premium is based on the salary used as the basis for social insurance premium payment. Specifically, the health insurance premium rate for employees and employers is as follows:

In total, employees and employers will pay 4.5% of the employee's monthly salary to the Social Insurance Fund and the Health Insurance Fund. Of which, employees pay 1/3 and employers pay 2/3.
Pursuant to Clause 3, Article 4 of Decree 146/2018/ND-CP and Clause 4, Article 12 of the Law on Health Insurance 2014. Students who are in the group of subjects paying health insurance with State budget support have the following payment levels:
The monthly health insurance contribution is equal to 4.5% of the basic salary. From July 1, 2023, the basic salary is 1,800,000 VND/month.
Therefore, the health insurance contribution rate for students for the 2023-2024 school year is 4.5% x 1,800,000 = 81,000 VND/month, equivalent to 972,000 VND/year.
Of which, students contribute 70%, equivalent to 56,700 VND/month, and the State budget supports 30%, equivalent to 24,300 VND/month.
Pursuant to the provisions of Point e, Clause 1, Article 7 of Decree No. 146/2018/ND-CP. The health insurance premiums for households in 2023 from July 1, 2023 are as follows:
1) The first person pays 4.5% of the basic salary, equivalent to 81,000 VND/month. In which the basic salary is 1.8 million/month.
2) The second person contributes 70% of the first person's contribution, equivalent to 56,700 VND/month.
3) The third person contributes 60% of the first person's contribution, equivalent to 48,600 VND/month.
4) The fourth person contributes 50% of the first person's contribution, equivalent to 40,500 VND/month.
5) From the fifth person onwards, the contribution is 40% of the first person's contribution, equivalent to 32,400 VND/month.
Social insurance contribution rates for other subjects are prescribed as follows:
- For people in near-poor households living in poor districts according to Resolution No. 30a/2008/NQ-CP dated December 27, 2008 of the Government and districts applying the mechanism and policies according to Resolution No. 30a/2008/NQ-CP, 100% support of health insurance premiums is implemented according to the provisions of Article 8, Decree 146/2018/ND-CP.
- For children under 6 years old, 100% of the cost of participating in health insurance is free and a free health insurance card is issued.
